SOLO Soccer Release & Waiver

RELEASE OF LIABILITY, ASSUMPTION OF RISK & LIKENESS CONSENT

SOLO Soccer Training Camp & Beach Blitz Soccer Tournament — Galveston, TX • May 26–31, 2026

This is a SOLO Soccer event and is not affiliated with LC United. In consideration of my child's participation in the SOLO Soccer Training Camp (May 26–29, 2026) and the Beach Blitz Soccer Tournament (May 30–31, 2026) (collectively, the "Event"), I, the undersigned parent or legal guardian, freely and voluntarily agree to the following:

1. ACKNOWLEDGMENT OF RISK
I understand that soccer, and particularly sand soccer, involves inherent risks that cannot be eliminated regardless of the care taken. These risks include but are not limited to: soft-tissue injuries from running on unstable sand surfaces; foot lacerations, blisters, or burns from sand, shells, or debris; sprains, fractures, concussions, and other contact injuries; heat-related illness including heat exhaustion and heat stroke; exposure to sun, wind, saltwater, and marine life; and any other injury or illness arising from athletic activity in a beach environment. I acknowledge that sand socks will be provided and are required during all sand play, but that no protective equipment eliminates all risk.

2. RELEASE & WAIVER
I, on behalf of myself, my child, and our heirs, estates, and assigns, hereby RELEASE, WAIVE, AND FOREVER DISCHARGE SOLO Soccer, Skylar Webber, and Shelby Webber (collectively, the "Released Parties") from any and all claims, demands, causes of action, liability, losses, or damages arising out of or related to my child's participation in the Event, whether caused by negligence of the Released Parties or otherwise. This release does NOT extend to the Beach Blitz Soccer Tournament organizers (Soccer Resort / Beach Blitz Soccer) or any other third party; those entities maintain their own liability and registration requirements.

3. MEDICAL AUTHORIZATION
In the event of an emergency where I cannot be reached, I authorize the Released Parties to consent to reasonable medical treatment, including first aid, CPR, or emergency transport, for my child. I accept responsibility for all associated medical costs.

4. VIDEO RECORDING & LIKENESS RELEASE
All SOLO Soccer training camps and events are recorded. SOLO Soccer uses this footage to build individual highlight reels within each player's SOLO Soccer profile. These highlight reels serve as official verification of the player's SOLO Skills Scores and are made available to the player and their family for personal use, including college recruiting purposes.

SOLO Soccer maintains national partnerships with college coaches, professional scouts, and competitive soccer recruiters. Players whose likeness is authorized may gain positive exposure through the SOLO Soccer website and recruiting platform. Additionally, likeness authorization is required for consideration for TEAM SOLO — the national SOLO Skills team, where selected players serve as positive soccer role models to players around the world.

☐ I GRANT PERMISSION — I authorize the Released Parties to use my child's name, image, voice, and likeness in video, photography, and other media for SOLO Soccer highlight reels, recruiting profiles, promotional materials, social media, and website content. I understand this allows my child to receive personal highlight reels, be featured on the SOLO Soccer platform for college recruiting visibility, and be considered for TEAM SOLO. This consent is ongoing and may be revoked in writing at any time.

☐ I DECLINE — While I understand that all events are recorded, I do not authorize the use of my child's likeness in any capacity. Any footage containing my child will be excluded during editing and deleted. I understand that by declining, my child will not receive personal highlight reels, will not have a college recruiting profile on the SOLO Soccer platform with SOLO Skills Score verification, and will not be eligible for TEAM SOLO selection.

IMPORTANT NOTE REGARDING LIKENESS AUTHORIZATION
Likeness authorization is optional. However, declining will exclude your child from receiving a SOLO recruiting profile, personalized highlight reels, player verification videos, game video review, and consideration for TEAM SOLO. SOLO Soccer's mission is to provide your player with high-level exposure and training opportunities. We do not sell your child's likeness to third parties. Your child's name, image, and likeness are used to support their soccer development, document their training progress, and increase their visibility to coaches and recruiters through the SOLO Soccer platform. By granting permission, you acknowledge that SOLO Soccer may use authorized likeness in promotional materials, website content, social media, advertising, and commercial media produced by or on behalf of SOLO Soccer. This consent is ongoing and may be revoked in writing at any time.

5. GOVERNING LAW & SEVERABILITY
This agreement is governed by the laws of the State of Texas. If any provision is found unenforceable, all remaining provisions shall remain in full force and effect. I acknowledge that I have read this document in its entirety, understand its terms, and sign it voluntarily.
